Website to engrave a mans necklace? Dog tag, with a hanging cross.?

I want to get my boyfriend something really nice for this birthday, and he's been wanting a necklace with a a dog tag, and a hanging cross. So it looks like two different pendants on one chain. And I want to get the dog tag engraved with his favorite verse on the back and a little note from me on the front. Where could I go to do this? Would Kay Jewelers or Jared Jewelry have something like this that's not over the top in price? Or is there an online site I could go to? I don't want it to look cheap, so I'm hopping to stay in the lines of 100$-200$. If there's something super nice for less, or something really nice for a little over 200 let me know. thanks!
